
Early Alert Form
The purpose of the
Faculty members who recognize
students that fail to exhibit successful college skills during the first six
weeks of the semester should complete the online Early Alert form. Students
will be contacted regarding their Early Alert, and will receive information
regarding campus resources to assist them. The student's advisor and other

This form is sent to Christina Conner, Retention Specialist, who will then
contact the student.
The Early Alert is NOT a Drop Notification; please use the Drop
Notification Form to drop a student from your class.
