TESTING CONTRACT

Please read carefully the rules and conditions for registering for

the Accuplacer test on the Levelland Campus

The Accuplacer test is a computerized test. The test taker should have basic computer skills. If you do not have basic computer skills, please check with the Testing Secretary and inquire about the THEA test.

CONDITIONS FOR TESTING

THE ACCUPLACER TESTING FEE IS NON-TRANSFERABLE AND NON-REFUNDABLE. IF YOU ARE LATE FOR THE TEST, OR IF A TEST DATE IS MISSED, YOU WILL HAVE TO PAY THE FEE AGAIN.

YOU WILL NOT BE ALLOWED TO TEST IF YOU DO NOT HAVE YOUR 2 (TWO) FORMS OF ID AND YOUR RECEIPT. NO EXCEPTIONS! You will be required to repay testing fees and schedule another testing date if denied admission to testing based on unacceptable forms of identification.

1. You must have an application on file with SPC before filling out a registration form

      for testing, this includes Dual Credit Students.

2. The proper testing fee MUST be paid at the time of registration.

3. YOU WILL NOT BE ALLOWED TO TEST IF YOU DO NOT HAVE YOUR 2 (TWO) FORMS

     OF ID AND YOUR RECEIPT.   NO EXCEPTIONS! You will be required to repay testing fees

     and schedule another testing date if denied admission to testing based on unacceptable forms

     of identification.

1st Required Identification: PHOTO I.D.

Acceptable forms for your Photo I.D. include:

Valid and Current Driver's License - NOT EXPIRED -or-

Valid and Current Year School Identification Card with Picture and Name -or-

School Yearbook Picture - Current Year Only - or-

Valid and Current Passport Picture - No older than 1 year - or-

Valid and Current Military I.D. -or-

Valid and Current Employee I.D.

2nd Required Identification: SOCIAL SECURITY NUMBER

Acceptable forms for your Social Security Number Documentation include:

Social Security Card -or-

Insurance Card with your Social Security Number on it - or-

Current High School or College Transcript - or-

Any other legal document containing your Social Security Number

4. Students must wait at least 2 weeks before retesting.
