Course Proposal Form - This forms requires the necessary information for categorizing the course within the core curriculum and, more importantly, how the course will align with the core objectives. For the box labeled assessment, identify the elements from the core curriculum rubric that can be evaluated in the learning activities. Please provide the revised departmental syllabus, which includes course learning and core objective learning outcomes, with this form.
Procedure Form - This form requires a description of the process for the course proposal form. For example, describe collaboration with colleagues within the department to reach decisions for course proposal form, provide details about any faculty development, and/or list information regarding consultation with colleagues at other institutions.
Faculty Support Form - This form will indicate faculty, which will teach the course, will implement the changes. Department chair signatures are required on all forms. If you need additional space for names and signatures, print this form twice.
